Juvederm in New Jersey
David Evdokimow, MD

Juvederm Ultra Injectable Gel is a smooth-consistency gel made with hyaluronic acid (HA). This filler is most frequently used to treat "parentheses," or nasolabial folds (the creases that run between the sides of your nose and the corners of your mouth). However, there are other applications, such as use in the lips or any area of volume loss. Juvederm binds easily to water, as the product breaks down it attracts more water at the site of the injection, thereby hydrating the skin and ensuring the longevity of the result. No skin test is required.

Juvederm Ultra Plus is similar to Juvederm Injectable Gel, but is made with larger molecules of HA. It can be used to correct deeper wrinkles and furrows and is well suited for lip enhancement. In Dr. Evdokimow's experience, results are longer lasting with Juvederm Ultra Plus than with Juvederm Ultra.

Before the Procedure

Before any injectable filler treatment, a thorough evaluation is performed. The patient's concerns and goals are discussed and a physical examination of the treatment area is performed. Other facial features will be assessed in order to maintain facial symmetry.

The choice of filler is determined based on the patient's goals and the desired area of augmentation, as well as the desired longevity of the results. Understanding the advantages and disadvantages of one filler compared to another as well as their limitations is essential to achieving optimal results and limiting adverse reactions.

The results after Juvederm treatment are volume dependent. A critical mass of injected material should be reached before the desired augmentation is achieved. Discuss your concerns with your doctor. If the amount suggested exceeds your expectations or you have financial or other restrictions, you might reconsider the procedure or opt for another type of filler.

The strong affinity of Juvederm to water makes it a risky choice in the border areas such as the lower lid-cheek junction. The thinner skin of the eyelid, with loose attachments to the underlying muscle, can respond to Juvederm injections with unwanted swelling, giving the eyes a puffy appearance when used for augmentation of the midface. Other fillers such as Sculptra and Radiesse should be considered when significant volume restoration is needed.

The same affinity to water makes Juvederm a superb choice for long lasting soft and natural-looking lip augmentation. Currently Juvederm and Juvederm Ultra Plus are fillers of choice for lip augmentation and lip definition in our New Jersey plastic surgery practice.

Patients can do their part to ensure positive results by discontinuing the use of blood thinners (such as NSAIDs, Coumadin and herbal remedies such as St. John's wort). If you are receiving treatments with Juvederm in New Jersey, you should also stop smoking and using nicotine products such as gum or patches. Smokers are treated only after they are made well aware about the increased risks for complications and prolonged recovery times.

Treatment with Juvederm Ultra and Juvederm Ultra Plus is frequently combined with other non-surgical treatments, such as Botox and Dysport. If treatment with Ulthera is planned after injectable fillers a grace period of 90 days should be given to ovoid interference with the longevity of the filler result. If possible, Ultherapy should be used prior to filler administration.

The Procedure

Dr. Evdokimow performs every injectable filler treatment himself at his office in Basking Ridge, New Jersey. Juvederm injections as well as other HA filler injections can be performed quickly and comfortably without local anesthesia. If desired, local anesthetic can be added to the filler to increase comfort. In sensitive areas like lips, nerve blockers can eliminate injection discomfort.

After the Treatment

After treatment, you may experience some redness or swelling at the injection site. This will subside quickly. If a minor bruising occurs, it is usually easily covered with makeup. Application of ice packs will help resolve the swelling. The use of NSAIDs should be avoided. Normal physical activities can be resumed the same day.

The duration of results depends upon the filler used, the anatomic area and the individual’s intensity of facial expression.

Juvederm Ultra/Ultra Plus usually last 6 to 10 months when used to fill wrinkles and 6 to 8 months when used for lip augmentation. The treatment should be repeated for continued improvement.
