About Facial Aging and Facial Rejuvenation

A young face is a full face. The skin and the deep structures are firmly attached to the facial skeleton with the help of ligaments. As time goes on, volume loss appears in all facial planes — bone, facial muscles, fat compartments, subcutaneous fat and skin. The volume loss begins when people are in their mid-twenties and progresses differently for each individual. Factors that contribute to facial aging are: gravity, facial expressions, sun damage, environmental influences, stress and smoking.

Facial aging consists of three major components:

  1. Loss of volume
  2. Descent and sagging of facial tissue
  3. Thinning and loss of skin elasticity of the face and neck.

The first signs of aging to appear include:

  • Descent of the corners of the mouth
  • Wrinkles around the eyes
  • Change of the position and shape of the eyebrows
  • Loose skin around the eyes (bags)
  • Deep creases around the nose, mouth and cheeks.
  • Jowl
  • Neck bands

Similar aging changes occur everywhere in the human body; the biggest difference is that facial fat usually decreases with aging, while the fat elsewhere in the body increases and becomes resistant to dieting and exercise.

Multiple minimally invasive procedures were design to reverse the visible signs of facial aging. Unfortunately, these were embraced by unqualified physicians because of their relative "simplicity" and overused and over advertised as a replacement to surgery. Of course, nothing can produce the dramatic results of facial surgery.

In the early stage of facial aging the minimally invasive procedures alone or in combination can produce impressive outcomes. Appropriate patient selection and respect to the specific indications and the limitations of the minimally invasive procedures are the key prerequisites of obtaining good results.

Ulthera®

In 2009, a revolutionary ultrasound based device received FDA approval for facial rejuvenation.

The procedure utilizes the same ultrasound technology that has been used in the medical community for more than 50 years.

Ulthera® is an ultrasound "see and treat" procedure. The target tissue is visualized with precision. Ultrasound waves thermally initiate the body’s own rejuvenation mechanism in the target tissue plain, producing tightening, lift and volume restoration. Ulthera® is usually used for eyebrow elevation and treatment of the entire face and upper neck. However, most recently, its clinical applications have started expanding to other body areas. The acquired clinical experience show that the device can be used for non-invasive rejuvenation and reverse early signs of aging of lower neck décolleté area, arms, breast, buttocks, periumbilical area of the abdomen, inner thighs and inner knees. The applications are expanding as more clinical experience is acquired.

Before the Procedure

The most important part of your consultation with Dr. Evdokimow will be to determine the degree of facial deep tissue and skin aging. In the face, the condition of forehead/eyebrows, eyelids, naso-labial folds, mouth, jaw border and neck are evaluated so that the final results preserve facial harmony.

In the body, the specific area of interest is evaluated for skin quality and subcutaneous fat deposits.

A detailed history of previous surgeries and minimally invasive procedures is taken because Ulthera® can interfere with the longevity of other minimally invasive treatments such as injectable fillers and Botox.

After listening carefully to your goals and concerns, and conducting a thorough physical exam, Dr. Evdokimow will develop a personalized treatment plan. Clinical photographs will be taken prior to your treatment.

Dr. Evdokimow will explain the precautions you can take to ensure safety comfort and optimal results.

The Procedure

The procedure is usually performed in the office in approximately 30 to 45 minutes. Mild sedatives and regional blocks (local anesthetics) significantly improve the comfort level.

During the procedure, the target tissue plains are selectively visualized and treated with precision. The first pass usually treats the deep supportive mechanism of the face and neck, achieving a lifting effect. A second, more superficial pass treats the deep layers of the skin, stimulating collagen production additional lift and volume restoration. Depending to the anatomy of the body area treated, the energy can penetrate as deep as the subcutaneous fat. Aggressive treatment of the subcutaneous fat can cause localized decrease of the fat volume. This observation is now used as additional evidence of the benefit of the treatment for a new segmental approach to facial and body rejuvenation, for example, in cases where the areas where preservation of fat volume is desired are excluded from the treatment or treated only in the superficial planes. When volume reduction is desired, aggressive treatment of the deep plains is performed.

There is not a unified opinion of the simultaneous use of Ulthera® and Botox. Some clinicians use the modalities in the same treatment. In my opinion, this compromises the predictability of the results.

If Ulthera® and Radiesse or hyaloronic acid (HA) fillers are used in the same treatment cession the Ulthera® treatment should be done first. If Sculptra Aesthetic is part of the treatment plan Ulthera® should be done first followed by Sculptra three months later. Repeated treatments with Ulthera® should be done at least three apart to take full advantage of the stimulated collagen production.

After the Procedure

The patient is free to go home immediately upon completion of the procedure. If oral sedatives are not used, driving is permitted at the time of the discharge. Visible swelling and redness disappears within 45 minutes. Occasional bruising and hyperpigmentation has been reported but is very rare and resolves spontaneously. The patient may resume normal physical activity including strenuous exercises immediately. Numbness of the skin and changes of sensation (tender to touch) may persist for two to seven days. Post-treatment pain is virtually nonexistent. Some patients report a feeling of tightness in the treated area.

The results are immediate and continue to improve for the next 90 days and are expected to last for about two years.
